Ambulatory Care: Primary and Specialty Care:

Bassett Healthcare Network's Ambulatory Care practices provide comprehensive outpatient care across a variety of primary care and specialty settings. Our clinics serve diverse patient populations ranging from preventive and wellness visits to chronic disease management and specialized treatment services. Licensed Practical Nurses (LPNs) are vital members of the care team, supporting providers, coordinating patient care, and helping patients achieve optimal health outcomes. Our Nursing Care/Magnet care delivery model focuses on empirical outcomes through transformational leadership, structural empowerment, exemplary professional practice, and new knowledge, innovations & improvements.

Long Term Care:

Bassett Healthcare Network's Long-Term Care services provide compassionate, resident-centered care in a supportive environment focused on quality of life, dignity, and individualized care planning. Our residents represent a diverse population with varying medical, rehabilitative, and long-term care needs. Licensed Practical Nurses (LPNs) play a critical leadership role in coordinating resident care, supervising direct care staff, and partnering with interdisciplinary team members to ensure the highest standards of care.

Ambulatory Care and Long-Term Care practices throughout Bassett Healthcare Network are hiring for Licensed Practical Nurses (LPNs) with various schedules, specialties, and locations available. Position Summary

The Licensed Practical Nurse (LPN), Ambulatory Care provides direct and indirect patient care under the direction of the Registered Professional Nurse, Nurse Practitioner, Physician Assistant, or Physician. The LPN functions as an integral member of the interdisciplinary health care team, assisting with the delivery of safe, high-quality patient care in the outpatient setting. This role supports patients across the lifespan through clinical assessment, patient education, treatment interventions, and care coordination activities.

    • Salary Range: $24.74 - $37.10 per hour
  • Licensed Practical Nurse (LPN), Ambulatory Care Advanced:
    • Salary Range: $25.73 - $38.69 per hour
  • Licensed Practical Nurse (LPN), Ambulatory Care Advanced Certified:
    • Salary Range: $26.77 - $40.55 per hour
  • Licensed Practical Nurse (LPN), Union
    • Salary Range: $24.71 – 35.71

The Licensed Practical Nurse (LPN), Long Term Care is responsible for coordinating, directing, and providing nursing care for residents in accordance with facility policies, professional standards of practice, and regulatory requirements. The LPN serves as a leader on the unit, collaborating with residents, families, providers, and interdisciplinary team members to ensure the delivery of safe, effective, and resident-centered care.

    • Salary Range: $24.71 – 35.71

Essential Job Functions Include:

  • Provides direct and indirect nursing care individualized to the patient's needs.
  • Collects, analyzes, and records relevant patient health data accurately.
  • Identifies actual and potential patient care needs and reports findings to the appropriate provider.
  • Implements and documents interventions as directed by the provider or supervising Registered Nurse.
  • Monitors patient responses to treatment and identifies deviations from expected outcomes.
  • Assists with examinations, procedures, and treatments within the LPN scope of practice.
  • Administers medications and treatments as prescribed and authorized by New York State regulations.
  • Adheres to approved, evidence-based practice standards and organizational policies.
  • Performs and documents Point of Care (POC) testing and quality control in accordance with established laboratory policies and procedures.
  • Maintains and documents testing reagents, supplies, and equipment according to established laboratory procedures.
  • Accurately documents patient care activities within the electronic medical record.
  • Assists in coordinating patient care, referrals, follow-up appointments, and communication with other members of the health care team.
  • Provides patient and family education consistent with the plan of care and scope of practice.
  • Advocates for patients to ensure care preserves and protects patient autonomy, dignity, and rights.
  • Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity.
  • Assures patients have informed consent completed for indicated procedures, as appropriate.
  • Functions within the New York State-defined LPN scope of practice at all times.
  • Maintains current licensure, certifications, and role-specific competencies.
  • Provides proof of current BLS validation to supervisor.
  • Completes required department credentialing according to organizational policy.
  • Provides evidence of completion of required annual training and competency validation.
  • Acquires and maintains current knowledge and skills in area of practice through continuing education and professional development.
  • Participates in quality improvement initiatives and supports efforts to enhance patient outcomes and patient experience.
  • Demonstrates consideration of patient safety, effective utilization of equipment and supplies, and cost-conscious care delivery.
  • Contributes to a collaborative team environment through effective communication and interdisciplinary teamwork.
  • Performs other duties as requested by supervisor or manager.

What we need from you

Education:

  • Graduate of a Practical Nursing program, required

Experience:

  • Previous healthcare experience, preferred

License/Certifications:

  • Current NYS LPN License and registration as a Practical Nurse or a valid interim permit, required
  • BLS certification must be obtained within 6 months of hire, required

We are also offering a sign-on bonus for this position! The bonus amounts are based on the position type and are listed below. Additional details and eligibility requirements will be discussed during the interview process. Bonus Amount:

  • RN (excluding management) sign-on bonuses are awarded in the amount of $20,000 for full time
  • LPN sign on bonuses are awarded in the amount of $10,000 for full time

Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. USD $37.10/Hr.
