Our residents represent a diverse population with varying medical, rehabilitative, and long-term care needs. Licensed Practical Nurses (LPNs) play a critical leadership role in coordinating resident care, supervising direct care staff, and partnering with interdisciplinary team members to ensure the highest standards of care.

Position Summary
The Licensed Practical Nurse (LPN), Ambulatory Care provides direct and indirect patient care under the direction of the Registered Professional Nurse, Nurse Practitioner, Physician Assistant, or Physician. The LPN functions as an integral member of the interdisciplinary health care team, assisting with the delivery of safe, high-quality patient care in the outpatient setting. This role supports patients across the lifespan through clinical assessment, patient education, treatment interventions, and care coordination activities.
The Licensed Practical Nurse (LPN), Long Term Care is responsible for coordinating, directing, and providing nursing care for residents in accordance with facility policies, professional standards of practice, and regulatory requirements. The LPN serves as a leader on the unit, collaborating with residents, families, providers, and interdisciplinary team members to ensure the delivery of safe, effective, and resident-centered care.
