Join a clinical team of RNs, LPNs, Clinical Office Assistants, and a Clinical Coordinator in the field of developmental disabilities. Work closely with residents, families, and teams to support healthcare decisions; provide staff support and education; manage acute/routine healthcare; collaborate with local providers; provide telephone triage and coordinated office/on-site hours. Responsibilities: Manage caseload per OPWDD guidelines; attend appointments and communicate results; deliver ongoing staff training; collaborate on medication management; provide hands-on care consistent with a home healthcare model; provide telephone triage/on-call rotation; maintain CEUs (approx. 6–10/year). Qualifications: Bachelor’s (BA/BSN) or Associate’s (ASN) with related experience. Required certifications: NYS Registered Professional Nurse License; valid NYS driver’s license; current CPR. Salary: Starting at $80,000, commensurate with experience.
