Join a comprehensive team of RNs, LPNs, Clinical Office Assistants, and a Clinical Coordinator in the field of developmental disabilities. Work closely with residents, families, and clinical teams to support healthcare decisions; provide staff support and education; manage acute and routine healthcare needs; collaborate with local healthcare providers; provide telephone triage with dedicated office and on-site hours. Responsibilities: Carry a caseload consistent with OPWDD guidelines; assist with appointments and communication; facilitate/maintain ongoing staff trainings; collaborate to manage medications; provide hands-on care consistent with a home healthcare model; provide telephone triage/on-call rotation; maintain 6–10 CEUs per year. Qualifications: Bachelor’s (BA or BSN) or Associate’s (ASN) with related experience. Required Certifications: NYS Registered Professional Nurse License; Valid NYS driver’s license; Current CPR certification. Annual salary starting at $80,000, commensurate with experience.
