The Community Habilitation Specialist (Per Diem) provides support in a home or community setting that promotes success at the highest level of independence possible. This includes teaching and reinforcing a wide range of daily living skills, promoting independence, preventing and responding to crisis situations appropriately, encouraging self-advocacy and facilitating the development of natural supports. All services will be provided in collaboration with program participants and their service plan, utilizing a person-centered approach.Qualifications- Required: Valid NYS license- Preferred: 1 year experience working with individuals with disabilities.Hourly rate starts at $16.50 and is commensurate with experience.
