The Program Specialist (Plan Writer) is responsible for the development and coordination of formal supports/services for individuals receiving applicable services at Pathfinder Village. Upholding individual rights and promoting person-centered outcomes while ensuring personal safety are essential hallmarks of this position. As a member of an individual's interdisciplinary team, the Program Specialist (Plan Writer) writes formal service plans, trains staff, attends meetings, ensures regulatory compliance, and acts as a liaison with care managers.

Qualifications:

Applicants must have at minimum, a high school diploma or equivalency AND at least one year of experience working with people with developmental disabilities. However, greater preference will be given to applicants who hold or are working towards a bachelor's degree in the human services field.
Occasional travel is required, valid NYS driver's license is required.
Excellent verbal and written communication is required.
Proficiency or knowledge of using a variety of computer software and email applications, especially Microsoft products.

Position starts at $18.50 an hour and is commensurate with experience
