Position Summary:
The Individual Program Coordinator (IPC) is responsible for the development and coordination of programming for individuals served at Springbrook. An IP Coordinator is responsible for individual plan writing across all settings, and ensuring accurate and consistent communication with all team members. The IP Coordinator plays a significant role in the assessment of quality and ensuring accurate documentation and electronic health records are maintained. Other important duties include development and delivery of training to peers for consistent implementation and sustainability of the program.
Principle Duties and Responsibilities for an assigned caseload:
- Facilitate the program development, implementation, and maintenance of each assigned resident
- Development and certification of Staff Action Plans: create, update and certify each assigned plan annually
- Maintain accurate individual records within the agency EHR
- Conduct POMS assessments per CQL guidelines
- Monitor the individual’s progress on a monthly basis with adjustments as necessary
- Assure accurate and consistent communication with all members of the individual’s team
- Coordinate with chosen program sites outside of the agency
- Assure all service provision meets regulatory requirements including the necessary paperwork completed in a timely manner
- Monitor services provision to assure the individual’s rights are not infringed upon without written justification and Human rights committee approval
- Assist with implementation of departmental and agency Consumer Satisfaction protocols
- All other duties as assigned.
Qualifications, Skills and Knowledge Requirements:
- Bachelor’s degree in a human services field, including but not limited to sociology, special education, rehabilitation counseling or psychology.
- At least one year of experience working directly with persons with intellectual disabilities or other developmental disabilities.
- Must be able to work a flexible schedule.
- Professional verbal and written communication is required.
- Proficiency in or knowledge of using a variety of computer software and email applications, especially Microsoft products.
- Meet the requirements to drive a Springbrook vehicle as necessary.
