Position Summary:
To ensure efficient programming, operation and compliance of the Day Habilitation program by providing training, coaching and supervision to Shared Day Habilitation Assistants. To ensure that training of staff, billing and provision of services comply with OPWDD and Medicaid regulation, Springbrook policies and procedures, and meets the Springbrook standard of excellence, incorporating best practices and a person centered approach which respects individual rights, choices and needs. This Includes maintaining a daily schedule of program activities that engage participants in choice making, community involvement, and onsite activities that assist the individuals with a structured and meaningful day.
Primary Duties and Responsibilities:
- Supervise the Shared Day Habilitation Assistant’s (SDHA) including mentoring, training, evaluations, participate in the hiring process, approval of time sheets, staff meetings, and facilitating teamwork.
- Provide 1:1 Supervisions monthly with each of the SDHA and document their progress and performance in the Day Force Management System.
- Demonstrate and role model positive leadership qualities and create a positive team dynamic within the Day Habilitation sites and departments.
- Assist in the development of the individuals Staff Action Plan and Behavioral Intervention Plans based on needs and choices of the individuals. Work cooperatively with the Assistant Director, Residence Supervisor, Nurses, Behavioral Specialist, Therapists and CCM.
- Maintain proper inventory of supplies needed to meet the needs of the individuals and the smooth operation of the program.
- Manage daily program schedule to plan for individual choice, community integration, clinical services, vocational training, recreational on and off-site activities, and volunteering.
- Maintain records for individuals in the day hab program.
- Monitor the daily activities of the staff and individuals to ensure that there is appropriate documentation of behavior plans, goals, meal time plans, and group notes.
- Communicate with the team to ensure that all safeguard documented in the Staff Action Plan as needed to ensure individual safety.
- Monitor and facilitate daily transportation to and from the Day Habilitation site and for offsite recreational activities.
- Monitor the Day Habilitation site for maintenance and equipment needs and report to the responsible party (work order).
- Complete all required Trainings facilitated by the Informational Technology Department and Staff Development.
- All other duties as assigned.
Qualifications, Skills and Knowledge Requirements:
- College degree preferred. May use a combination of education and/or experience that meet the requirements for this position.
- Three (3) years’ experience working with individuals with developmental disabilities.
- Minimum of one (1) years supervisory experience preferred.
- Must be able to work a flexible schedule.
- Duties require professional verbal and written communication skills.
- Proficiency in or knowledge of using a variety of computer software and e-mail applications, especially Microsoft Excel, Outlook and Word; have the aptitude to learn other computer software as necessary.
- Must be able to lift 50 lbs. independently and assist in lifting weights exceeding 50 lbs.
- Meet the requirements to drive Springbrook vehicles as required.
- Be able to perform all physical interventions.
