Position Summary:
Actively contribute as a team member to support the effective and seamless operation of the Day Habilitation Program. Key responsibilities include developing and maintaining a structured daily routine, providing personal care assistance, and creating a supportive, inclusive atmosphere both onsite and in the community. This role promotes meaningful daily experiences by encouraging individual independence and providing appropriate protective oversight. Adherence to all Springbrook policies and applicable state regulations is essential to ensure a safe, compliant, and efficient program environment.
Primary Duties and Responsibilities:
- Serve as a positive role model in all environments and interactions with the individuals you support.
- Advocate for the rights, preferences, and needs of the individuals in every setting.
- Demonstrate punctuality and reliability; consistent staff presence is essential to the well-being of those we support.
- Collaborate with individuals to observe, discuss, and assess their strengths and areas for growth in order to help develop personalized goals and support plans.
- Encourage and empower individuals to make informed choices about their goals, daily activities, and lifestyle.
- Tasks may range from providing full assistance with personal care to offering supervision and guidance, depending on each individual's needs and capabilities.
- Support individuals in building social connections and engaging in meaningful community involvement, including but not limited to:
- Participation in spiritual and recreational activities (e.g., swimming)
- Follow all individualized plans, educational materials, and goal-oriented activities as outlined.
- Accurately collect and record all required data.
- Complete all billing documentation for services rendered in a timely and accurate manner.
- Obtain and maintain all necessary training, certifications, and a thorough understanding of relevant policies and procedures.
- Maintain safe, clean, and organized environments—including physical spaces, vehicles, and shared areas.
- Participate in team meetings and scheduled one-on-one supervisory meetings.
- Perform all other duties as assigned.

Qualifications, Skills and Knowledge Requirements:
- High school diploma or GED required; Associates degree preferred.
- Experience working with individuals with developmental disabilities preferred.
- Duties require professional verbal and written communication skills.
- Proficiency in or knowledge of using a variety of computer software and e-mail applications, especially Microsoft Excel, Outlook and Word; have the aptitude to learn other computer software as necessary.
- Must be able to lift 50 lbs. independently and assist in lifting weights exceeding 50 lbs.
- Meet the requirements to drive Springbrook vehicles as required.
- CDL preferred, but not required.
- Be able to perform all physical interventions.
