- Ensure that the medical needs of people receiving services are met by collaborating with physicians, health care providers, and interdisciplinary team members. Follow through on the provision of nursing care by monitoring services and interventions.
- Safeguard the health, welfare, and safety of people receiving services by directing, coordinating, and supervising medical care planning, in alignment with OPWDD and Springbrook policies and procedures.
- Ensure proper staffing and oversight by training, monitoring, and evaluating staff authorized to administer medications in compliance with OPWDD and Springbrook requirements.
- Maintain continuity of care by:
- Conducting assessments across service settings
- Reviewing and responding to reports on individuals’ health status
- Supporting staff in carrying out treatments and interventions
- Determining when to contact physicians or escalate concerns
- Coordinating services between shifts
- Provide education and communication to people receiving services and their families, guardians, and staff, answering questions and anticipating needs.
- Maintain collaborative relationship with community partners through timely communication in addressing medical needs.
- Keep appropriate personnel informed of significant or complex medical situations and document all care and services per OPWDD and Springbrook standards.
- Serve as a role model in the completion of all required nursing documentation.
- Participate in Springbrook’s Administrator-on-Call rotation to provide oversight and decision-making support outside of regular business hours.
- Contribute to continuous quality improvement by identifying and implementing strategies that support safe, effective, person-centered nursing services.
- Perform all other duties as assigned.
